Hartley House

Salary Info:

Salary is $50,000 and the position is a regular F/T, Exempt employee. These items are non-negotiable. We offer a generous benefits package including medical and dental, four weeks of vacation, three personal days, and seven sick days and other perks


Hartley House is a historic settlement house serving the Hell’s Kitchen neighborhood in New York City since 1897. Programs include after school and summer day camp programs for children, support services for neighborhood seniors.


The Director of the Home Outreach Program for Elders (HOPE) is looking for an energetic, responsible, and conscientious Social Worker Supervisor to provide a full range of comprehensive case management activities in the HOPE program and providing supervision to case manager, support specialist and interns.  This person will further advance the ability of HOPE to meet the needs of seniors by providing direct services by providing comprehensive case management, bill pay assistance, in-home assessments, health and entitlement referrals, caregiver support, crisis intervention and support, and client advocacy.   S/he will work closely with HOPE director and with up to 35 regular clients and provide further referrals and temporary support to more per year.  Our approach is holistic and highly flexible so that we are able to meet the client at the level of care and need they present throughout service.


Specific Responsibilities
  • Conduct biopsychosocial assessments for new clientsCoordinate and monitor services, including home care, medical and/or psychiatric care, etc.
  • Assist with money management including monthly bill pay, budgetary assistance, and applying for private stipendiary assistance as necessary
  • Eligibility screening for public benefits/entitlements and facilitating the collection of documentation to support these applications
  •  Advocate on behalf of clients in matters relating to well-being such as tenant/landlord issues, legal affairs, and access to benefits
  • Maintain database records that document personal profiles, important contacts, and narrative accounts of interactions with clients and others
  • Crisis intervention when necessary
  •  Responsible for coordination of protective measures in cases of abuse or exploitation
  • Provide caregiver support
  •  Provide information and referral services to individuals that are not referred as HOPE clients
  • Increase quality of care by participating in the implementation of additional scientifically validated assessment tools and evidence-based practice models to achieve successful client outcomes
  •  Supervises case manager, support specialist, interns and volunteers
  • Conduct weekly staff meetings
  • Represents the program at inter-agency meetings
  • Attend community meetings for the purpose of conducting program outreach and trainings designed to build knowledge
  • Represents the agency to Clinton Cares neighborhood partnership
  • Serves as resource to Hartley House community in aging matters


Essential Skills:

  •  High proficiency in Excel, Microsoft Office Suite 2010, and Google Apps
  •   Knowledge of or QuickBooks a plus
  •  Superior writing and highly professional communication skills
  •  Bi-lingual (Spanish/English) Preferred
  •  Ability to work both independently and in a team environment


  • MSW Required
  • Strong references
  • General knowledge of public benefits and entitlements
  • Experience with aging population strongly preferred.
