PROGRAM: Sunnyside Community Services (SCS), an innovative community-based, social service organization in Queens (10 minutes by subway from Grand Central), is seeking an experienced Supervisor to join our SCS Caregiver Support - Western Queens Program. The program provides information, referrals, counseling, and respite to family caregivers - those family members or friends who are providing care to physically disabled or cognitively impaired older adults, grandparents caring for grandchildren and older adults caring for a disabled adult. Respite service provides temporary relief from caregiving responsibilities on a limited basis and can include in-home care or adult day care services. POSITION DUTIES AND RESPONSIBILITIES:
As the Supervisor, you will work under the supervision of the Program Director to mentor, guide and support the team of four Caregiver Support Specialists (CSS) who are working under your leadership. You will provide weekly one-on-one supervision and group supervision to four CSSs and use your social work expertise to guide them in providing the best client care management, including ongoing reassessment of client needs, provision of services, and monitoring of service efficacy. You will oversee the appropriateness of referrals, care plans, and overall service delivery to clients, and review documentation in the Department for the Aging's database, Peer Place. You will coordinate client training presentations and participate in outreach to the community. You will be responsible for ensuring no gaps in service to clients in the absence of a CSS and at times provide direct service in support of clients.
