Intake Coordinator/Case Manager – Bilingual English/Spanish

Salary range $50,000 and $52,000

Description:

Job Summary: The Intake Coordinator/Case Manager (IC/CM) will be a part of a newly formed Care Connections team that will screen all new potential clients for their eligibility to any one of our nine senior services at SCS. This will require knowledge of all available programs and resources in the agency to properly direct a potential client to the appropriate services. The IC/CM will then do a complete screen and intake for our Department for the Aging funded Case Management program which includes services such as Home Delivered Meals, EISEP Home Care and Case Management services. The IC/CM will also carry a caseload of APS clients. The IC/CM will work under the supervision of the Case Management Program Director and with day-to-day direction of the Care Connections Manager. Good record keeping, computer knowledge and time management skills are essential.

RESPONSIBILITIES:

  • Assist with screening of clients in need of services
  • Conduct intakes and make referrals to other available resources
  • Identify and assist clients with basic information on Benefits and Entitlements
  • When necessary, contact emergency services when a client is in distress
  • Carry as caseload of APS clients- adhere to DFTA standard procedures and APS protocols established to provide effective services to the at-risk senior population
  • Conduct and record periodic communication with APS workers to determine the status of clients
  • Input all required data in multiple data systems as required by DFTA and SCS
  • Participate in trainings and conferences and agency meetings
  • Assist with outreach in the community

QUALIFICATIONS:

  • Experience in Human Service field
  • Good verbal and written communication skills- must be bi-lingual in Spanish and English
  • Must have excellent interpersonal skills with an ability to establish rapport with staff, clients, informal supports and other community and government agencies
  • Must have minimum bachelor’s degree; or HS diploma or Associates degree with four (4) years of casework experience in a community service or social action program or RN qualification
