POSITION SUMMARY: Sunnyside Home Care Project is responsible for ensuring the provision of safe and appropriate care to the clients it serves in accordance with New York State Department of Health and contractual requirements and current standards of care. The role of the Acute Care Aide (ACA) is responsible for helping to fulfill that objective by
ESSENTIAL DUTIES AND RESPONSIBILITIES
The Acute Care Aide may also perform other duties, as assigned.
CARE ACTIVITIES
Performs home visits to provide direct care to clients according to the client’s plan of care.
COMMUNICATION ACTIVITIES
• Develops and maintains effective relationships with clients/family members and home care staff.
• Utilizes observation skills, asking questions for details and clarity and reporting information (OAR) regarding the client’s health and home environment to the nurse and the home care coordinator appropriately and timely.
• Directly informs the nurse of changes in the client’s condition affecting the plan of care.
• Observes and reports information regarding client/family members’ satisfaction with home care services and overall well-being.
• Adheres to agency policies and procedures.
• Always displays professional behavior as a representative of Sunnyside Home Care Project in the community.
• Completes all required client related documentation in accordance with agency policy.
• Promptly notifies the team of client related issues/concerns that may create a potential safety hazard and/or negative impact on the client’s care.
PHYSICAL REQUIREMENTS:
Frequent and prolonged standing, walking, lifting, pushing and/or transferring clients; lifts supplies and equipment, frequently twists, stoops, bends, squats; occasionally climbs stairs. May spend varied percentage of working time indoors or outdoors depending on assignment needs and communicates orally and/or in writing with client’s families, nurses, other team members, including office staff etc.
WORKING CONDITIONS:
Works without direct supervision in client’s homes and may be exposed to elements in the client’s home; is subject to varying and unpredictable situations with frail, compromised, elderly or sick individuals.
Available to work a flexible schedule up to twelve (12) hours per day, six (6) days per week, including holidays. Able to accept client assignments within the agency’s geographic service area.
