INTRO/PROGRAM DESCRIPTION: The Manhattan Outreach Consortium provides outreach and housing placement services to adults who are street homeless. The Uptown office services the Upper West and Upper East Sides of Manhattan and collaborates with various community partners and healthcare agencies throughout Manhattan. Office is located in the Upper West Side. 

PURPOSE OF POSITION: Provide assistance, support and supervision to Uptown Goddard’s outreach team.

Roles, Responsibilities, and Essential Duties

Staff Supervision
  • Provide clinical supervision for Housing Outreach Specialists; conduct weekly supervision to review client status and goals for assigned HOS staff
  • Assist in selection, training, evaluation and discipline of staff

Oversee Staff Case Management
  • Monitor & case managers delivery of services for homeless adults, with a goal of placing them in transitional and permanent housing
  • Support outreach team in the performance of client intake, clinical assessment, service planning, advocacy, referral and escort services to assist in housing placement
  • Ensure staff enter and update client case records appropriately and in a timely manner and as prescribed by Agency Policy
  • Provide follow-up case management for clients placed in housing, to promote retention and document out comes
  • Communicate with evening/weekend staff and with staff of other agencies to coordinate client service
3)  Collaborate within and across agencies
  • Schedule and attend monthly Case Conferences at Safe Havens to review the status and progress of UG clients towards housing
  • Participate in Case Conferences with HOS Case Managers to discuss individual clients as needed
  • Attend Community Board Meetings on behalf of Uptown Goddard upon request of Program Director
  • Participate in outreach trips to identify and communicate with  homeless clients
  • Assist Outreach staff in canvassing, Joint Operations, DHS Clean-ups as needed
  • Participate in Annual HOPE Count and 4 overnight counts each year
5)    Manhattan Consortium team member
  • Attend staff meetings, case conferences, training programs and supervision meetings; participate in program evaluation and improvement
6)    Other duties as required Qualifications/Educational Requirements
  • LMSW license or Master’s in Counseling required, MPA or MPH may be considered
  • 2-3 years previous supervisory experience
  • Prior clinical experience with homelessness, mental health, and/or substance abuse
  • Ability to multi-task/work in a fast-paced environment
  • Participate in 4 overnight counts each year
  • Proficiency in Spanish and English preferred
  • Valid driver’s license and a driving record that meets the agency’s auto insurance guidelines (including no suspensions within the last 3 years)
  • Skills, Knowledge and Abilities
  • Strong leadership skills
  • Proficient in Microsoft Office
  • Physical Requirements
  • Frequent walking and standing
  • Occasional lifting of up to 30lbs
Work Environment
  • Indoors and outdoors; all weather

Salary Information

Full-time; Salaried
Days: Monday - Friday
Hours: 8:00am – 4:00pm
Salary Range: Commensurate with experience
