Casa Mutua, located on East 102nd Street, is our 54-unit permanent supportive housing residence for formerly homeless single adults with histories of mental illness and substance abuse. We are seeking a Supportive Housing Social Worker to join our team to promote the independence and stability of our tenants by providing comprehensive supportive services.

 

Reporting to the Casa Mutua Director, the Supportive Housing Social Worker will be responsible for providing direct therapeutic supportive services to tenants including assessment, supportive counseling, case management, advocacy for entitlements and services, group work and referral to resources including psychiatric and medical care, civil legal services, substance abuse services, etc. The Supportive Housing Social Worker will work directly to provide person centered services to assigned tenants many of whom have been diagnosed with Major Depression, Bipolar Disorder, Schizophrenia and Anxiety.  Additionally, the Supportive Housing Social Worker will be expected to coordinate psychiatric, medical and substance use services as needed to ensure excellent service delivery.  H/she will be expected to participate in a teamwork approach to service provision, and work cooperatively with staff at all levels at Casa Mutua and throughout Lenox Hill Neighborhood House to ensure quality service provision. The Supportive Housing Social Worker will also be responsible for completing comprehensive assessments including psychosocials to inform, develop and implement service plans, participating in case conferences and staff meetings, escorting tenants to appointments as needed, assisting tenants in problem-solving and conflict resolution, and attending and facilitating group activities including monthly parties, meals and groups to model appropriate social behavior and build relationships with tenants. The Supportive Housing Social Worker will be expected to perform other related duties incidental to the work as assigned.

 

Qualifications:

The successful applicant must possess excellent interpersonal, engagement, and relationship building skills. A social justice perspective combined with a commitment to the well-being of mentally ill and homeless populations is required.  Experience working with mental illness, substance use and trauma informed care and treatment are strongly desired.  Experience working in a residential environment a plus.  The successful candidate must also be detail-oriented and have excellent communication, computer and organizational abilities. Masters of Social Work, Masters of Counseling or other relevant Master’s degree preferred; Bachelors of Social Work with relevant experience will be considered.  Spanish proficiency preferred.
