Our Geriatric Care Management Program provides comprehensive care management services to thousands of frail and homebound older adults on Manhattan’s East Side. Our Geriatric Care Management Program serves the ever-growing population of older adults on the East Side, linking care managers, social workers, lawyers, and other providers together in our efforts to support the frailest members of our community.

 

RESPONSIBILITIES: Reporting to the Geriatric Care Management Program Director, the Supervising Social Worker will:
  • Supervise five Geriatric Care Managers (LMSW and BA level staff), Social Work interns and volunteers who are providing care management services to frail and homebound older adults;
  • Review case files and provide quality assurance to ensure compliance with the highest standard of care;
  • Conduct and coordinate staff trainings and educational workshops on issues relevant to older adults aging in place;
  • Conduct intake and engage with homebound older adults and their caregivers as needed to ensure their stability and safety. The Social Worker Supervisor may also carry a small caseload of older adults who need ongoing assistance; and
  • Work collaboratively with Neighborhood House staff from a variety of departments and disciplines as well as our community-based and government partners.
 
QUALIFICATIONS:
The successful Social Work Supervisor possesses a minimum of three years of Social Work experience, particularly in the field of older adults. S/he should possess excellent social work and clinical skills; expertise in accessing appropriate resources in the community; and have an understanding of the health care system and how it affects older adults. S/he must be detail oriented and have strong administrative skills. S/he takes significant initiative and demonstrates excellent communication, time management, computer and organizational abilities. LMSW or related Master's level degree (e.g. Mental Health Counseling) required. SIFI certification a plus. A competitive salary with comprehensive benefits package available.
