Lenox Hill Neighborhood House, widely recognized as one of New York’s premier human services providers, is a 123-year-old settlement house that provides an extensive array of effective and integrated services--social, educational, legal, health, housing, mental health, nutritional and fitness--which significantly improve the lives of 15,000 people in need each year, ages 3 to 103, on the East Side of Manhattan. Our constituents include older adults, homeless and formerly homeless adults, children and families, recent immigrants, disabled persons, adult learners and thousands more. Reporting to a Lenox Hill Neighborhood House Geriatric Care Social Worker, and working closely with the Community Health Worker Program Manager at New York Presbyterian- Weill Cornell Medical Center, the Community Health Worker Coordinator will work as part of an interdisciplinary management team to improve the health outcomes of our community members.


  • Managing caseload of clients recently discharged from the hospital and coordinating post-discharge community care designed to prevent unnecessary re-hospitalization;
  • Assessing client needs to determine appropriate interventions and providing necessary assistance, including making medical appointments, accompaniment to appointments and linkages to community resources;
  • Timely maintaining and inputting accurate case records into electronic client database;
  • Referring clients to community and hospital services, such as senior and community centers, ongoing care management services, outpatient medical clinics, mental health services, support groups and health and wellness workshops;      
  • Performing individualized support and information sessions and community-based education workshops;
  • Coordinating and supervising interns and volunteers;
  • Managing the evaluation of client outcomes, challenges and successes;
  • Working closely with project leadership to develop and implement new health-related initiatives;
  • Providing support, training and guidance to  Community Health Workers; and
  •  Participating in community and hospital councils, committees and work groups


The successful Community Health Worker Coordinator will demonstrate a commitment to serving community members with health and other challenges; have excellent communication skills; have the ability to work independently and as part of an interdisciplinary team; be comfortable conducting hospital and home visits; and be passionate about improving the health outcomes of recently discharged clients so they can remain in the community. Prior Community Health Worker experience preferred. Spanish fluency preferred.

Salary Information

Salary range: low 40,000s. Comprehensive benefits package.
