CLUBHOUSE GENERALIST

Job Description

Intro/Program Description: The Other Place is a clubhouse that assists individuals with behavioral health to achieve goals in the areas of working, living, learning, and socializing. Members belong to a community where they are empowered through participation in all clubhouse activities and operations. Members work side by side with staff.

 

Purpose of Position: The position of Clubhouse Generalist will work in collaboration with staff and members in the daily operation of the Clubhouse which provides a safe, nurturing, in a non-institutional setting for behavioral health that fosters independent living and greater participation in the community.

 

Roles, Responsibilities, and Essential Duties

1.      Provide individual rehabilitation counseling and case management

·        Maintain a caseload of 20 adults with serious and persistent mental illness

·        Conduct functional assessments and develop service plans and reviews using a person-centered approach

·        Assist members with establishing and achieving rehabilitation goals

·        Referrals to vocational, educational, social, and housing opportunities/resources

·        Collaborate with treatment, housing, education, and vocational providers

·        Active participation in operating and maintaining clubhouse

·        Assist in organizing and engaging members in social and recreation activities

·        Co-facilitation of evidenced-based workshops/meetings

2.      Documentation

·        Complete daily/monthly progress notes

·        Prepare Annual Service Plan

·        Prepare Quarterly Service Plan Reviews

·        Prepare Discharge Summaries if indicated

·        Utilize AWARDS electronic system for all documentation

 

3.      Coaching

·        Assist members with skill development

 

4.      Other duties as required

 

Qualifications/Educational Requirements

·        Bachelors Degree in Social Work, Psychology or Mental Health Counseling required

·        2 years experience working with individuals with mental illness

·        Strong Computer  skills

·        Excellent verbal and written skills

·        Spanish Speaking preferred

 

Skills, Knowledge and Abilities

·        Effective communication, (verbal and written), organization, time management, knowledge of mental illness and substance use, AWARDS, Microsoft word, excel, and the ability to coach others and be reflective.

Physical Requirements

·        Ability to lift up to 30 lbs

 

Work Environment

·        Members and staff share office

·        Program is located in basement office, weekend social and recreation off-site.

Salary Information

Days/Hours: Monday to Friday, some weekends
Hours: 9:00am to 5:00pm
