BronxWorks is hiring 1 Case Manager for the Supportive Housing Department.

The Brook Supportive Housing Program has multiple opportunities for Case Managers to join an established clinical team. The Brook permanently houses 189 tenants; the majority are formerly street homeless and have serious mental illness, chronic illness (including HIV/AIDS) and substance abuse challenges.  Case Managers provide psychosocial support, medication monitoring, financial management assistance and supportive case management services to single adults in a community environment. 

Ideal candidate is able to demonstrate ability to work in partnership with clients from a strengths based perspective, overcoming stigma, and navigating complex systems. Additionally, ideal candidate is able to adapt to an active environment, is exceptional with time management, has strong writing and communications skills, exhibits excellent attention to detail and remains a team player.

The Brook provides a high level of support to its Case Managers through individual supervision, rounds, on-site trainings and staff meetings. Some evenings and Saturdays required. BronxWorks offers opportunities for continuing education and career growth.

RESPONSIBILITIES

  • Manage cases for concrete services such as access to benefits, case management assistance and medical care coordination.
  • Provide psychosocial support to program clients.
  • Conduct medication monitoring for clients in need of assistance.
  • Perform financial management assistance for client support.
  • Provide supportive case management services to single adults in a community environment.  
  • Work some evening hours and Saturdays as required.
  • Perform additional duties as assigned by manager.

 

QUALIFICATIONS

  •  Bachelor’s Degree in Social Work, Psychology, Sociology, Human Services required.
  • A minimum of at least 1 year of experience working with individuals who have experienced street homelessness, severe and persistent mental illness or substance abuse.
  • Demonstrated ability to work in partnership with clients from a strengths based perspective to overcome stigma, and navigate complex systems.
  • Ability to adapt to an active work environment while being a team player.
  • Bilingual English and Spanish language proficiency preferred but not required.
  • Engaging personality and detailed orientated skills are essential.
  • Strong oral and written communication, time management and organizational skills are necessary.
  • Proficiency in Microsoft Office suite and other standard business technology is required.

 

PHYSICAL REQUIREMENTS

  • Ability to use a computer for prolonged periods.
  • Ability to occasionally lift and/or move up to 10 pounds.
  • Ability to stand, walk, or sit for long periods of time.
  • Ability to climb five flights of stairs, if required to conduct home visits and/or fieldwork.
  • Ability to bend and retrieve objects and/or documents.
  • Ability to travel in the boroughs of New York City and its adjacent counties via public transportation.
  • Ability to travel for a minimum of 1 mile within a reasonable amount of time and in all types of weather, including inclement weather, if required to conduct home visits and/or field work.

 

Advanced-level professional competency in written and spoken English language is required. Authorization to work in the United States for any employer is mandatory.
