Job Description

Overview: The Door is a multi-service youth development agency serving the needs of young people between the ages of 12 and 21. The Member Services Department is responsible for welcoming new members, conducting brief interviews / risk assessments with, and connecting them to appropriate and wanted programs and services based on their needs and interests. 

 

The Care Coordination model is an innovative service program in conjunction with Health Homes, the New York State medical redesign program, primarily intended to support adolescents and young adults who are impacted by trauma, mental health or other medical issues.

 

The Care Coordinator is responsible for assisting youth with improving in their overall stability and functioning in daily life, including progress toward and achieving person centered goals. Ongoing training and professional supervision is provided to the candidate who fills this position.

 

Responsibilities:

·        Conduct regular outreach to young people

·        Ensure that youth receive referrals to appropriate Door and community resources

·        Assess young person and develop service plan

·        Facilitate service delivery

·        Coordinate treatment plans with service providers

·        Maintain all reporting requirements on timely basis: agency, Health Homes and regulatory in multiple electronic case record systems

·        Coordinate and facilitate case conferences, including staff from all program areas/agencies to determine appropriate plans for young people

·        Provide de-escalation and crisis intervention, as necessary

·        Participate in agency-wide and department staff meetings monthly; as well as all scheduled team meetings

·        Complete any additional responsibilities and functions as assigned by their supervisor

 

Qualifications

·        BA in related field and four years work experience, preferably in a mental health program, or a Master’s degree in related field and two years work experience

·        Demonstrated ability to engage hard-to-reach populations

·        Familiarity and experience with trauma informed approach

·        Experience and/or knowledge working with street-involved youth

·        Knowledgeable of and ability to apply positive youth development principles to working with young people

·        Understanding of  adolescent development, assessment, counseling and/or crisis management

·        Experience and comfort with reporting to multiple electronic databases

·        Excellent oral and written communication skills required

·        Proficiency in Spanish, French or Mandarin strongly preferred

·        Ability to work effectively and creatively as part of an interdisciplinary team

Salary Information

Salary: low 40’s, commensurate upon experience
Hours: Full time (afternoon and evening hours)
