Job Description

JOB SUMMARY:         To ensure that the residents at the senior housing can lead independent, healthy and active lives within their community by providing a safe, healthy, and friendly living environment; responding to the cultural and developmental needs; and providing services that improve the quality of life for all seniors.

 

EDUCATION, EXPERIENCE AND SKILL REQUIREMENTS:

 

·     Bachelor’s Degree Preferred; Master’s Degree in Social Work/Human Services Preferred

·     Bilingual English/Mandarin or Cantonese, some Spanish conversation is a plus

·     Previous experience working with older adults population

·     Excellent communication and organization skills

·     Writing skills in Chinese & English

 

ESSENTIAL DUTIES AND RESPONSIBILITIES:

 

Resident Services:

·       Conduct biopsychosocial assessments that includes mental health screening to identify emerging physical and psychosocial needs

·       Provide case management, case assistance and care coordination for all residents

·       Provide additional supportive services that include individual supportive counseling, family counseling, telephone reassurances, weekly home visits, service referrals and crisis interventions

·       Plan & implement the cultural activities and create monthly calendar of events:

o   Social: monthly birthday celebrations, bingo, mahjong, neighborhood trips, etc.

o   Cultural Events: Three Kings, Moon Festival, Lunar Chinese New Year; Dragon Boat Festival, Mother’s Day, Father’s Day, Summer Picnics, Thanksgiving Luncheon,  Christmas and New Year’s Parties, etc.

o   Recreational and Educational activities: Yoga/Tai Chi, Dancing, ESL, Arts & Craft; Computer Club, Walking Club, Gardening Club, Newspaper Club; Intergenerational Activities with our child care programs; etc.

o   Health workshops: Nutritional, Medical, Blood Pressure Screenings, Emergency Preparedness (i.e. fire safety); etc.

·       Recruit and support a cohort of senior volunteers that will assist in planning and organizing program activities, meet monthly with volunteers and floor captains

·       Work in collaboration with the Building Property Manager and Custodian to ensure that residents are able to abide by HUD regulations

·       Coordinate weekly meetings with management team, keep notes and follow up on issues

·       Work in collaboration with the Property Manager to schedule and facilitate regular tenant meetings

·       Work in collaboration with community partners to extend the health and medical services and referrals

·       Work collaboratively with the all the GSS senior programs to develop activities and special events

·       Maintain daily case notes, monthly program services reports and case files with up-to-date client information

·       Prepare bi-annual social services presentations to the governing board

·       Supervise Social Work Interns

·       Special Projects Assigned

·       BEST/NORC- serve as a back-up support as needed

Salary Information

REPORT TO: Managing Director SALARY RANGE: $42,000-$45,000
WORK SCHEDULE: Monday-Friday, 9am-5pm (or 8am-4pm)
